FTM Top Surgery

Condition: Transgendered men often have an undesired female chest configuration. Male chest construction is designed to correct this problem.

Procedure: Very small breasts can be dealt with by subcutaneous mastectomy consisting of removal of breast tissue through a small "keyhole" incision around the areolar margin. More commonly, however, there is too much skin excess for this limited procedure and a "double incision" mastectomy is required. This attempts to allow the underlying muscular shape to predominate. It also requires nipple grafts, a form of skin graft. Careful downsizing and placement of the nipples is mandatory for the best aesthetic result.



Consequences: Loss of nipple sensation is universal with the grafting technique and possible with the keyhole procedure. The scars of a double incision procedure are usually obvious and always permanent.

Risks: As with all surgery there are risks of infection, fluid collection, bleeding, numbness, poor healing and undesirable scars. As with all bilateral procedures asymmetry may occur. Small amounts of breast tissue remain, and while the procedure may decrease the risk of breast cancer it is not entirely eliminated.

McLean Clinic Top Surgery: The clinic is a popular site for this procedure, with clients often coming long distances. We do our best to make the experience as easy as we can for all. The procedure is performed under general anaesthetic (you are asleep) as day surgery (no hospital stay) and takes about 2 hours. Drains or tubes are not required in most of our double incision procedures. A responsible adult must accompany you home and stay with you for at least 24 hours. You may be out of bed. Typically, you are seen the following day and can bathe after the initial dressing change. Perhaps the greatest hassle is the need to protect the nipple grafts, and to keep the nipple dressings dry until they are removed after 2 weeks. We recommend that you take pain killers regularly for the first two days so that pain is prevented rather than treated.
